2 Dental Cleanings
Two routine dental cleanings per membership year, based on the patient’s individual dental needs and eligibility.
1–2 Dental Examinations
One to two periodic dental examinations per membership year, as recommended by the dentist.
1 Set of Dental X-Rays
One routine set of diagnostic dental X-rays per membership year, as clinically appropriate.
2 Fluoride Applications
Two fluoride applications when recommended by the dentist and appropriate for the patient’s age and dental needs.

Is this dental insurance?
No. The Park Place Dental In-House Dental Membership Plan is a dental membership/discount plan and is not dental insurance. Benefits are available only at Park Place Dental and are not transferable to another dental office.
How long does membership last?
Membership is valid for 12 months from the date of enrollment. Plan fees, benefits, and terms are subject to change upon renewal.
How do payments work?

Membership fees must be paid according to the payment terms established by Park Place Dental.

Patient portions for services are due at the time services are provided unless other arrangements have been approved by the practice.

Can I combine it with insurance or other discounts?
The membership plan cannot be combined with dental insurance, other discount plans, or promotional discounts unless specifically authorized by Park Place Dental.
What treatments or services may be excluded?

Services provided by outside specialists, hospitals, laboratories, or other third parties are not eligible for the membership discount unless specifically approved by Park Place Dental.

Cosmetic and elective procedures may be excluded from the 20% treatment discount.

Preventive services included in the membership are subject to clinical necessity and the patient’s individual treatment needs.
